1. Scope
This Privacy Policy describes how the CVI Peak Prevention Program (“CVI,” “we,” “our”) collects, uses, and discloses information in connection with the peak-prevention.crownvalleyimaging.com website (the “Site”) and the intake and inquiry services offered through it. Protected Health Information (PHI) collected in connection with actual imaging services is governed by our separate HIPAA Notice of Privacy Practices, provided at intake.
2. Information we collect
- Information you provide: name, business email, organization, preferred tier, and optional notes submitted through the Request Private Access form.
- Automatic information: standard server logs such as IP address, user agent, referring URL, and timestamp used to detect abuse, operate rate limiting, and maintain security.
- Cookies and analytics: if enabled, we use first-party analytics to measure aggregate Site performance. We do not sell personal information.
3. How we use information
- Respond to intake inquiries and coordinate scheduling.
- Operate, secure, and improve the Site.
- Detect, prevent, and respond to fraud, abuse, or security incidents.
- Comply with legal obligations and enforce our Terms.
4. How we share information
We do not sell personal information. We share information only with (a) our affiliated clinical and operational personnel on a need-to-know basis; (b) service providers such as email delivery and hosting who are contractually bound to safeguard it; (c) authorities when required by law; and (d) successors in the event of a corporate transaction, subject to continued protection.
5. HIPAA and medical information
Information submitted through this Site is generally not Protected Health Information under HIPAA, because an intake request does not establish a treatment relationship. PHI collected during actual Program services (clinical intake, imaging, reports) is subject to HIPAA and our Notice of Privacy Practices, which is provided at the time of service. Please do not submit personal medical details through this website.
6. Your choices
- You may decline to submit the intake form.
- You may request access to, correction of, or deletion of the information you submitted through the Site by contacting our concierge team.
- California residents have specific rights under the California Consumer Privacy Act (CCPA / CPRA). We honor those rights where applicable and will not discriminate against you for exercising them.
7. Data security
We use reasonable administrative, technical, and physical safeguards to protect information. No method of transmission or storage is 100% secure; we cannot guarantee absolute security.
8. Retention
We retain intake information for as long as needed to respond to inquiries and to comply with our legal obligations, after which it is deleted or de-identified.
9. Children
The Site and Program are intended for adults. We do not knowingly collect personal information from children under 18.
10. Changes
We may update this Privacy Policy from time to time. The “Last updated” date indicates when the policy was most recently revised. Material changes will be highlighted on the Site.
